In this screen you indicate which host is used in the given activity. If the pathogenicity class has already been defined for the host, it will be automatically adopted. If it is not filled in, you can still fill it out. Plants and animals have pathogenicity class 0. When a host appears on list A1 of Annex 2 or on the list of Annex 4, the field 'On list A1 of appendix 2 or Annex 4' is filled in automatically.

You also indicate according to which article in appendix 5 the host is classified (e.g. 5.2 or 5.4 (without the letter)). See appendix 5 (See this link for the flowchart) you can also use the the abbreviated appendix 5 (part 1 and 2).

Finally, you can make comments or an explanation to the host.
